Living individuals who accessed a CVS digital property—including the CVS website, CVSHealth.com, or the CVS app—in the United States before 2026-07-27 may qualify, unless they excluded themselves. Physical-store-only visits do not qualify.
Submit a claim online through the official settlement website using the login ID and PIN from the notice, or complete and mail the official claim form to CVS Digital Privacy Settlement, PO Box 1110, Costa Mesa, CA 92628. Online claims are due by 11:59 p.m. EST on 2026-11-16; mailed claims must be postmarked by that date.
