Living, natural persons residing in the United States whose private information was affected by the FinWise data incident occurring on or about May 31, 2024, including individuals who received a data-incident notice. The affected information may include names, dates of birth, Social Security numbers, and customer account numbers.
Submit a completed Claim Form online through the settlement administrator's claim portal using the Unique ID and PIN from the notice, or complete and sign a paper Claim Form and mail it to FinWise Data Incident, Settlement Administrator, P.O. Box 4390, Portland, OR 97208-4390. Online claims must be submitted, and mailed claims must be received or postmarked, by 2026-10-29.
