Generally, a person is eligible if they underwent bariatric surgery at a Charleston Area Medical Center facility by Dr. Robert B. Shin or Dr. Samuel Rossi between 2015-04-05 and 2025-04-04, and had a hiatal hernia repair during the same surgery. Final eligibility depends on the court-approved settlement terms and administrator records.

No claim form is required for the Guaranteed Payments Fund; remain in the settlement and do not opt out.
